There are several items that are needed for DNA sample collection:
Participation Consent Form
Table to record participant family history
Sticker sheet of Genographic ID numbers
Cheek Swabs (Whatman Omni Swabs)
Sample storage tube (see item 1 in the figure to the right)
Information cards for participants
Sample Collection Procedure
Have the Subject read and sign the Participation Consent Form.
Record the birth places of the participant's 4 Grandparents.
Place a Genographic ID sticker on a sample storage tube (item 5 above).
Place the same ID sticker on an information card. Make sure this Genographic ID sticker corresponds to the ID number next to the participant's information on the family hisory table.
If the participant has eaten, drank or smoked recently, have them rinse their mouths out with water.
Open the cheek swab, at the corner by number "4" in the figure to the right. Be careful to not touch the swab.
Have the participant take the swab at number "3", and scrape the inside of the cheek vigorously with the brush, indicated by the number "2" in the figure. Scrape each cheek 6 times.
Open the sample storage tube, being careful to not spill the liquid contents of the tube.
Place the brush tip into the tube, and eject the brush into the tube. To do this, hold onto the number "3", and press in the number "4" portion of the stick.
Close the tube, and dispose of the rest of the swab.
Give the participant teh information card and a copy of the Participation Consent Form.
